Report a Conduct Violation

Silence enables harm. If you have witnessed a violation of the VITALIS Code of Professional Conduct, submit a report. Every submission is reviewed by the independent Certification Governance Board.

WHAT HAPPENS NEXT

The Reporting Process

We take every report seriously. Here is exactly what happens from the moment you submit to the final resolution.

Submission

Your report is received by the Governance Board intake team within 24 hours. You receive an acknowledgment with a confidential case number.

Preliminary Review

The Board conducts a preliminary review within 5 business days to determine if the allegation falls within the scope of the Code.

Investigation

If the allegation is within scope, a formal investigation begins. Both parties are given opportunity to provide evidence and witness statements.

Resolution

The Board renders a decision within 45 days of case opening. Disciplinary actions are communicated to all relevant parties with full transparency.

Reporter Protection Policy

VITALIS Lab prohibits retaliation against any individual who submits a good-faith report. Your identity is held in strict confidence throughout the investigation process. Anonymous reports are accepted, though they may limit our ability to follow up for additional evidence. False or malicious reports may result in disciplinary action against the reporter.
